Full Certification
To assign you course work for a scuba diving certification you will need all of these forms completed and correct. We can not allow you in the water to train until all the forms have been signed by all required parties.

Registration: This form is for contact information and can be downloaded HERE

Liability: This form is a basic liability release and can be downloaded HERE

Medical: This form verifies that you are healthy enough to dive and can be downloaded HERE

Youth release: Minors can not sign for themselves and this form is confirmation that a parent or legal guardian has cosigned all the other forms and can be downloaded HERE

Responsible diver: This is a code of values we expect you as a diver to uphold, and can be downloaded HERE
